THE MAJOR BENEFACTORS HONOR AWARD NOMINATION FORM
Sponsored and Administered by THE ASSOCIATION FOR LIBRARY TRUSTEES AND ADVOCATES (ALTA) a division of The American Library Association (ALA)
The Major Benefactors Honor Award was established to give recognition to individuals, families or corporate bodies who have made major benefactions to public libraries. The award, announced at the ALA Annual Conference and presented locally, consists of two specially prepared citations for the recipient(s) and the beneficiary library. The significance of the gift will be measured from the point of view of the recipient library, rather than by the size of the gift alone.
REQUIREMENTS FOR NOMINATION INCLUDE:
o Benefits to the library in the form of money, real or personal property, negotiable paper or other tangible contributions.
o Endorsement by the board of trustees of the library involved.
o An accompanying fee of $100.00 to cover administrative overhead.
o Six (6) copies of the completed application and accompanying support materials.
